Indication
: Tx
of major depressive disorder (MDD);
tx of binge-eating and vomiting in patients with moderate-to severe bulimia
nervosa; obsessive-compulsive disorder (OCD);
premenstrual dysphoric disorder
Mechanism
of action :
Inhibits CNS neuron 5-HT reuptake; minimal or no effect on reuptake of NE or DA;
Pregnancy Category C
Contraindication
:
Severe renal or hepatic failure; hypersensitivity;
lactation; concomitant MAOIs or within 2 wk of MAOI withdrawal.
Precaution
:
Unstable epilepsy, liver and renal impairment,
cardiac disease, diabetes, electroconvulsive therapy, bleeding disorders,
closed-angle glaucoma; pregnancy.
Drug
Interaction : Tightly
protein bound drugs , diazepam , clopidogrel ,MAOIs,
Li+
Side
Effect : Nervousness, insomnia, anxiety,
headache, tremor, drowsiness, dry mouth, nausea, vomiting, sweating, diarrhea.
Seizures, mania, hypomania . Hyponatraemia; elevation of hepatic enzymes.
Monitoring
parameters : Mental status for depression, suicidal ideation, anxiety, social
functioning, mania, panic attacks; akathisia, sleep status; blood glucose (for diabetic patients), baseline liver
function
Usual
Dose :
- Antidepressant or Antiobsessional agent or Premenstrual dysphoric disorder
--- Oral, initially 20 mg (base) a
day as a single morning dose. The dose may be increased as needed and tolerated
at intervals 4-8wk
- Antibulimic agent --- Oral, the
target dose is 60 mg (base) a day as
a single morning dose. Patients may need to be titrated up to this dose over
several days.
