Indication
: Edema
of cardiac origin, hepatic cirrhosis w/ ascites, HTN
Mechanism
of action :
Amiloride HCl inhibit Na+ reabsorption in the late distal convoluted
tubules, connecting tubules, and collecting ducts in the kidneys.
Hydrochlorothiazide increases excretion of Na+ and Cl- in approximately equivalent
amounts.

Category
B
Contraindication
:
Hyperkalemia, other concomitant antikaliuretic therapy or K supplementation,
renal insufficiency
Precaution
: Impaired
hepatic & renal function, monitor for signs of fluid & electrolyte
imbalance. May impair glucose tolerance, cause hyperkalemia or hypokalemia,
precipitate hyperuricemia &/or gout. Increase in cholesterol &
triglycerides
Drug
Interaction : Alcohol, antidiabetics, antihypertensives, corticosteroids,
ACTH, skeletal muscle relaxants, nondepolarizing drugs, NSAIDs, lithium
Side
Effect :
Headache, weakness, nausea, anorexia, rash & dizziness orthostatic effects
Monitoring
parameters :
assess weight, I&O reports daily to determine fluid loss; BP, serum
electrolyte, BUN, creatinine
Usual
Dose : Initially,
1-2 tab daily. Max: 4 tab. Should be taken with food.
Missed
Dose :
Take the missed dose as soon as you remember. However, if it is almost time for
your next dose, skip the missed dose and take only the next regularly scheduled
dose. It is not necessary to take a double dose.
Auxiliary
labels : This medicine
causes frequent urination. This drug should not be taken at bedtime.
Do
not eat salty foods and high salt or sodium foods. Do
not take this medicine with potassium supplements or high potassium food.
